ABSTRACT

O objetivo deste estudo foi revisar as ferramentas diagnósticas e de manejo da colangite aguda, da tríade de Charcot aos critérios de Tóquio. Foi realizada revisão da literatura no PubMed® utili- zando a combinação dos descritores "Tokyo Guideline", "Char- cot's triad" e "Acute cholangitis". A tríade de Charcot foi a pri- meira ferramenta diagnóstica da colangite aguda. Apesar de sua contribuição, foram os critérios de Tóquio a primeira diretriz com evidência clínica para diagnóstico, classificação de severidade e estratégia para o manejo da colangite aguda. A literatura atual busca identificar marcadores de mau prognóstico do paciente, como a procalcitonina, assim como o tempo ideal para a terapia antibiótica e a drenagem biliar. É bem estabelecido o quanto a tríade de Charcot, juntamente dos critérios de Tóquios, contri- buiu para o melhor diagnóstico e manejo da colangite aguda. Todavia, mais estudos são necessários para validação científica dessas diretrizes na prática clínica-cirúrgica.


The objective of this study was to review acute cholangitis diag- nostic and management tools, from Charcot's triad to Tokyo guidelines. The keywords "Tokyo guideline", "Charcot's triad", and "Acute cholangitis" were searched in PUBMED® and used to review the literature. Charcot's triad was the first diagnostic tool in acute cholangitis. Despite its contribution, Tokyo guidelines were the first evidence-based parameters for diagnosis, severity assessment, and strategy for acute cholangitis management. The current literature searches for biomarkers of patient's bad prognostic, such as procalcitonin, as well the optimal timing of antibiotic therapy and biliary drainage. The good contribution Charcot's triad and Tokyo guidelines has made to improved diagnosis and management of acute cholangitis is well-esta- blished. However, further studies are required for scientific vali- dation of these guidelines in clinical-surgical practice.

ABSTRACT BACKGROUND: Obstructive jaundice may lead to ominous complications and requires complex diagnostic evaluations and therapies that are not widely available. OBJECTIVE: To analyze the epidemiological profile, referral routes and diagnostic accuracy at admittance of cases of acute cholangitis among patients with obstructive jaundice treated at a referral unit. DESIGN AND SETTING: Cross-sectional study at a tertiary-level university hospital. METHODS: Patients with obstructive jaundice who were treated by means of endoscopic retrograde cholangiopancreatography, resection and/or surgical biliary drainage were evaluated. The main variables analyzed were epidemiological data, referral route, bilirubin levels and time elapsed between symptom onset and admittance and diagnosing of acute cholangitis at the referral unit. The accuracy of the clinical diagnosis of acute cholangitis was compared with a retrospective analysis on the medical records in accordance with the Tokyo criteria. RESULTS: Female patients predominated (58%), with an average age of 56 years. Acute cholangitis was detected in 9.9% of the individuals; application of the Tokyo criteria showed that the real prevalence was approximately 43%. The main referral route was direct contact (31.8%) and emergency care (29.7%); routing via official referral through the public healthcare system accounted for 17.6%, and internal referral from other specialties, 20%. The direct route with unofficial referral was the most important route for cases of neoplastic etiology (P < 0.01) and was the fastest route (P < 0.01). CONCLUSIONS: There is a deficiency in the official referral routes for patients with obstructive jaundice. The accuracy of the clinical diagnosis of acute cholangitis was poor. Wider dissemination of the Tokyo criteria is essential.

ABSTRACT

Objective: Biliary tract infections include cholangitis and cholecystitis. They are associated with high morbidity and mortality in elderly patients with co-morbid disease. The present study was undertaken to determine the microbial aetiology causing biliary tract infections and also to study their antimicrobial resistance profile. Materials & methods: A retrospective study was conducted from January 2011 to December 2016 at the Enteric Diseases Division, Kasturba Medical College Hospital, Manipal. Patients with biliary tract infections admitted in tertiary referral health care hospital, Manipal were included for the study. Aerobic and anaerobic bacteriological and fungal aetiology of biliary tract infections were recorded along with their antimicrobial resistance profile. Results: Out of 307 bile samples sent for aerobic culture and susceptibly testing 187 (60.91%) were positive for culture, of which Escherichia coli (44.4%) was the predominant aetiology followed by Klebsiella pneumoniae (27.3%). Among the 14 samples sent for anaerobic culture, 5 (35.75%) specimens showed growth, of which Bacteroides fragilis group was found to be the predominant anaerobe. Among the 201 bacterial pathogens tested for their antimicrobial susceptibility, 108 (53.73%) isolates were resistant, out of which 9 were PDR Enterobacteriaceae with 12 ESBL strains. All the Candida species were susceptible to fluconazole with the exception of C. glabrata and C. krusei. All the anaerobic isolates were found to be susceptible to Metronidazole. Conclusions: The high rate of bacterial infection particularly gram-negative bacteria was recorded. It is necessary that antimicrobial therapy be initiated when culture or the clinical conditions reports caution. Routine aerobic and anaerobic culturing of bile samples with biliary tract infections are imperatively necessary. With the emergence of multidrug resistant pathogens and change in the microbiological spectrum of biliary tract infections, there is a need for the empirical antimicrobial therapy in every clinical setting.


Objectivo: Las infecciones del tracto biliar incluyen colangitis y colecistitis. Se asocian a gran mortalidad y morbildiad en pacientes ancianos y con comorbilidad. El presente studio se hizo para detemrianr la etiologia microbiana que produce infecciones biliares y para estudiar su perfil de resistencia antimicrobiana. Materiales & metodos: Se hizo un studio retrospectivo entre los meses de Enero 2011 a Diciembre de 2016 en la "Enteric Diseases Division, Kasturba Medical College Hospital, Manipal" en India. Los pacientes con infección de vías biliares admitidos al centro de atención de tercer nivel se incluyeron en el estudio. Se buscaron bacterias aerobicas y anaerobicas y etiologia fungica y se analizó su perfil de resistencia antibiotica. Resultados: De 307 muestras de bilis enviadas para cultivo aerobico y antibiograma, 187 (60.91%) crecieron en el medio de cultivo, predominando Escherichia coli (44.4%) seguida por Klebsiella pneumoniae (27.3%). Entre las 14 muestras analizadas en medio anaerobio, 5 (35.75%) mostraron crecimiento de Bacteroides fragilis. Entre 201 bacterias probadas por antibiograma, 108 (53.73%) tuvieron perfil de resistencia, de los cuales 9 fueron PDR Enterobacteriaceae con 12 cepas ESBL. Todas las especies de Candida fueron susceptibles al fluconazol con la excepción de C. glabrata y C. krusei. Todos los aislados anaerobios fueron susceptibles al Metronidazol. Conclusiones: Se encontró una alta tasa de infección bacteriana con predominio de gram-negativos. Se hace necesario iniciar terapia antimicrobiana cuando lo sugieren las condiciones clínicas o el resultado del cultivo. El cultivo rutinario de bilis es imperioso. Dado el aumento de patógenos multirresistentes se requiere inicio empírico inmediato

ABSTRACT

A colângio-hepatite é considerada uma causa frequente de insuficiência hepática em gatos e é classificada em neutrofílica, linfocítica e esclerosante. Os objetivos deste estudo foram determinar a frequência de colângio-hepatite em gatos diagnosticados na Região Metropolitana de Porto Alegre, descrever seus aspectos anatomopatológicos e estabelecer uma associação com as infecções por Escherichia coli, vírus da imunodeficiência felina (FIV) e vírus da leucemia felina (FeLV). No período de janeiro de 2000 a julho de 2016 o Setor de Patologia Veterinária da Universidade Federal do Rio Grande do Sul realizou 1915 necropsias de gatos, destes, 32 foram diagnosticados com colângio-hepatite, representando 1,7% dos casos. Destes, a colângio-hepatite linfocítica (CHL) foi diagnosticada em 68,7% (22/32), a neutrofílica (CHN) em 21,9% (7/32) e a esclerosante (CHE) com 9,4% (3/32). A idade variou de quatro meses a 16 anos, com a mediana de seis anos, acometendo predominantemente gatos sem raça definida. Somente na CHN observou-se predisposição por machos, verificado em 85,7% (6/7) dos casos. Enterite e pancreatite foram identificadas concomitantemente com a colângio-hepatite em 56,2% (18/32) dos casos, cada, e a formação de tríade foi identificada em 46,9% (15/32) dos gatos. Através da imuno-histoquímica, 68,2% (15/22) dos gatos com CHL, foram positivos para FIV, 40,9% (9/22) para FeLV e 31,8% (7/22) marcação para ambos os retrovírus. Na CHN, 85,7% (6/7) positivos para FIV, 57,1% (4/7) para FeLV e 42,8% (3/7) imunorreação para os dois retrovírus. Na CHE, 100% (3/3) dos casos apresentaram marcação para FeLV, 33,3% (1/3) para FIV e 33,3% (1/3) para ambos. Imunomarcação para E. coli foi observada em 27,3% (6/22) dos casos da CHL, 28,6% (2/7) da CHN e em 33,3% (1/3) da CHE. E. coli, Enterococcus sp. e Klebsiella pneumoniae foram os micro-organismos mais frequentes isolados no exame bacteriológico. A visualização da E. coli, através da IHQ no sistema hepatobiliar de gatos diagnosticados com colângio-hepatite associados à inflamação, sugere que a doença se desenvolveu secundariamente à infecção bacteriana ascendente.(AU)


Cholangiohepatitis is considered a frequent cause of liver failure in cats, and are classified as neutrophilic, lymphocytic and sclerosing. The aims of this study was to determine the frequency of cholangiohepatitis in cats diagnosed in the metropolitan region of Porto Alegre, to describe the anatomopathological aspects and to establish an association with Escherichia coli, feline immunodeficiency virus (FIV) and feline leukemia virus (FeLV). From January 2000 to July 2016, the Department of Veterinary Pathology of the Universidade Federal do Rio Grande do Sul performed 1915 cat necropsies, of which 32 were diagnosed with cholangiohepatitis, representing 1.7% of the cases. Of these, lymphocytic cholangiohepatitis (LCH) was diagnosed in 68.7% (22/32), neutrophilic (NCH) in 21.9% (7/v) and sclerosing cholangiohepatitis (SCH) with 9.4% (3/32) of the cases. In general, age ranged from four months to 16 years, with the median age of six years, and predominantly affected cats with mixed breed. Only in NCH demonstrated male predisposition, verified in 85.7% (6/7) of this cases. Enteritis and pancreatitis were identified concomitantly with cholangiohepatitis in 56.2% (18/32) cases, each, and triad formation was identified in 46.9% (15/32) of cats. In the immunohistochemistry, we observed that 68.2% (15/22) of the cats with LCH were positive for FIV, 40.9% (9/22) for FeLV, and 31.8% (7/22) positive for both retroviruses. In NCH, 85.7% (6/7) was positive for FIV, 57.1% (4/7) for FeLV, and 42.8% (3/7) immunoreactions for the two retroviruses. In SCH, 100% (3/3) of the cases presented FeLV marking, 33.3% (1/3) for FIV and 33.3% (1/3) for both. Immunostaining for E. coli was observed in 27.3% (6/22) of LCH, 28.6% (2/7) of NCH, and 33.3% (1/3) of SCH. E. coli, Enterococcus sp. and Klebsiella pneumoniae were the most frequently microorganisms isolated in the bacteriological examination. The visualization of E. coli by the immunohistochemistry in the hepatobiliary system of cats diagnosed with cholangiohepatitis suggests that the disease developed secondary to ascending bacterial infection.(AU)

ABSTRACT

La colangitis biliar primaria (CBP), es una colangiopatía crónica caracterizada por la destrucción selectiva de las células epiteliales biliares de conductos hepáticos de pequeño y mediano calibre, que afecta principalmente a mujeres. Los principales síntomas son la fatiga y el prurito, sin embargo, gran porcentaje de los pacientes pueden ser asintomáticos. El diagnóstico se basa en anticuerpos antimitocondriales (AMA) con títulos >1:40, fosfatasa alcalina >1,5 veces del límite superior normal por más de 24 semanas e histología hepática compatible con la patología. Se asocia con múltiples enfermedades principalmente de carácter autoinmune extra hepáticas, enfermedades tiroideas, óseas, entre otras. El tratamiento de primera línea es el ácido ursodesoxicólico (AUDC) que a pesar que no cura la enfermedad, mejora las pruebas del perfil hepático, así como el retraso en la progresión a cirrosis. Actualmente se encuentran en estudio nuevos tratamientos y terapias adyuvantes. El propósito de esta revisión es ofrecer una actualización de este tema que se presenta en los servicios de medicina interna y gastroenterología; para su realización se conformó un equipo interdisciplinar que desarrolló una búsqueda en la base Medline a través de PubMed con los tesauros correspondientes y se procedió a una lectura crítica y analítica de títulos, resúmenes y textos completos para el filtro, extracción y síntesis de la información encontrada


Primary biliary cholangitis (PBC) is a chronic autoimmune cholangiopathy characterized by a selective destruction of biliary epithelial cells of small and medium caliber hepatic ducts, which mainly affects women. The main symptoms are fatigue and pruritus, however, a large proportion of patients may be asymptomatic. The diagnosis is based on AMA titers >1:40, alkaline phosphatase >1.5 times the upper limit for more than 24 weeks and compatible liver histology. It is associated with multiple autoimmune diseases mainly extrahepatic, thyroid diseases, bone diseases, among others. The first line treatment is ursodeoxycholic acid (UDCA), that improves liver function tests and delay the progression to cirrhosis. Currently, there are new treatments and adjuvant therapies on study. The purpose of this review is to offer an update in this topic, which is very important in gastroenterology and internal medicine. We formed an interdisciplinary team to search in the data base Medline thorough PubMed with the keywords describe below, we made a critical lecture of the titles and abstracts of each article to write this paper

ABSTRACT

La colangitis aguda puede definirse como el síndrome caracterizado por signos y síntomas de infección sistémica con origen en el árbol biliar. Se produce por la presencia de bacterias en la bilis junto a un aumento de la presión intrabiliar por una obstrucción, siendo la coledocolitiasis la causa más frecuente. El tratamiento de esta entidad se basa en la combinación del drenaje biliar y la administración de antibióticos. Esta entidad infrecuente en gestantes, aparece en una gestante con 35,4 semanas de gestación, que llega a la institución de salud por fiebre referida, vómitos, dolor epigástrico e íctero, se le realiza ultrasonido abdominal constatando dilatación de vía biliar y leucograma alterado con predominio de los segmentados, con stab y gránulos tóxicos; lo cual permite realizar diagnóstico de colangitis. Se realizó una colangiopancreatografía retrógrada endoscópica mediante la técnica convencional, con la paciente en decúbito lateral izquierdo oblicuo. Se utilizó arco en C y las imágenes fueron interpretadas por el endoscopista, con un mínimo de radiación. Se realizó canulación selectiva de la vía biliar y opacificó, observando dilatación de las vías biliares intrahepáticas, colédoco dilatado de 15mm, en cuyo interior se observa defecto de lleno ovoide. Se realizó esfinterotomía amplia y se extrajo cálculo pardo de 1cm de diámetro y varios litios pequeños, acompañados de abundante contenido biliar y pus blanquecino. Posterior a la colangiopancreatografía retrógrada endoscópica la paciente evoluciona de forma satisfactoria.


Acute cholangitis may be defined as the syndrome characterized by signs and symptoms of systemic infection originated in the biliary tree). The presence of bacteria in the bile together with the increment of the intra-biliary pressure due to an obstruction produces it; choledocholithiasis is the most frequent cause. The treatment of this entity is based on the combination of the biliary drainage and antibiotics administration. This entity, infrequent in pregnant women, appears in a pregnant woman with 35.4 weeks of pregnancy, who visited the health institution referring fever, vomits, epigastric pain and jaundice. An abdominal ultrasound showed biliary way dilatation and the leukogram was altered with predominance of the segmented ones, stab and toxic granules, allowing arriving to the diagnosis of cholangitis. An endoscopic retrograde cholangiopancreatography was carried out using the conventional technique, with the patient in oblique right lateral decubitus. An arc in C was used and the endoscopist interpreted the obtained images; with a minimum radiation, a selective cannulation of the biliary way was made and made it opaque, observing dilatation of the intra-hepatic biliary ways, dilated common bile duct of 15 cm. Inside it, it was observed an ovoid defect of filling; a wide sphincterotomy was developed and a brown calculus of 1 cm of diameter and several little ones, accompanied by abundant biliary substance and whitish pus were extracted. After the endoscopic retrograde cholangiopancreatography the patient had a satisfactory evolution.

ABSTRACT

Coledocolitíase refere-se à presença de cálculos no ducto biliar comum e é secundário à passagem de cálculos da vesícula biliar para o ducto colédoco. A associação dos critérios clínicos, laboratoriais e ultrassonográficos determina sensibilidade de 98% para o diagnóstico de coledocolitíase. A sensibilidade para detectar cálculos segundo os exames de imagens pouco varia entre as técnicas disponíveis, representando acurácia de 90% para ultrassonografia abdominal. A medicina avança continuamente através de novos meios de diagnóstico. À medida que novas tecnologias surgem, anamnese e exame físico aprofundado são desvalorizados. Quanto mais exames forem solicitados, maior a chance de resultados falso-positivos, e resultados mal interpretados podem ocasionar adiamentos e cancelamentos de procedimentos essenciais. O relato refere-se a uma paciente do sexo feminino apresentando icterícia, febre, dor em hipocôndrio direito, transaminases excessivamente elevadas e ultrassom abdominal compatível com cálculo na vesícula biliar e material no colédoco terminal. O relato objetiva evidenciar que a integração entre história, exames físicos e laboratoriais é fundamental para se determinar quais exames adicionais solicitar. A paciente apresentava quadro de coledocolitíase e colangite evidenciados ao exame físico e testes complementares. Entretanto, solicitou-se colangiorressonância ­ desnecessariamente ­ para confirmação diagnóstica a uma paciente sem condições financeiras para investigação adicional. É indiscutível que os modernos recursos tecnológicos proporcionam meios necessários para diagnóstico mais preciso. Deve-se basear em critérios clínicos, laboratoriais e ultrassonográficos na investigação de coledocolitíase. Os exames são como a própria denominação se dá (complementares), e eles servem para auxiliar, complementar o raciocínio médico, e não para, sozinhos, realizar o diagnóstico (AU)


Choledocholithiasis refers to the presence of stones in the common bile duct and is secondary to the passage of calculi from the gallbladder to the common bile duct. The association of clinical, laboratory and ultrasonographic criteria determines a 98% sensitivity for the diagnosis of choledocholithiasis. The sensitivity to detect stones according to imaging varies little across the available techniques, representing an accuracy of 90% for abdominal ultrasonography. Medicine advances continuously through new diagnostic means. As new technologies emerge, anamnesis and in-depth physical examination are devalued. The more tests are requested, the greater the chance of false-positive results, and misinterpreted results may lead to delays and cancellations of essential procedures. The report refers to a female patient with jaundice, fever, right hypochondrium pain, excessively elevated transaminases, and abdominal ultrasound consistent with stone in the gallbladder and material in the terminal bile duct. The aim of the report is to show that the integration between history, physical and laboratory tests are fundamental to determine which additional tests to request. The patient had a picture of choledocholithiasis and cholangitis evidenced on physical examination and complementary tests. However, cholangio resonance was requested ­ unnecessarily ­ for diagnostic confirmation to a patient who could not afford further investigation. It is indisputable that modern technological resources provide the means needed for more accurate diagnosis. The investigation of choledocholithiasis should be based on clinical, laboratory and ultrasonographic criteria. The tests are like the denomination itself (complementary), and they serve to aid, complement the medical reasoning, and not to make the diagnosis alone (AU)

ABSTRACT

Sclerosing cholangitis is a spectrum of chronic progressive cholestatic liver disease characterized by inflammation, fibrosis, and stricture of the bile ducts, which can be classified as primary and secondary sclerosing cholangitis. Primary sclerosing cholangitis is a chronic progressive liver disease of unknown cause. On the other hand, secondary sclerosing cholangitis has identifiable causes that include immunoglobulin G4-related sclerosing disease, recurrent pyogenic cholangitis, ischemic cholangitis, acquired immunodeficiency syndrome-related cholangitis, and eosinophilic cholangitis. In this review, we suggest a systemic approach to the differential diagnosis of sclerosing cholangitis based on the clinical and laboratory findings, as well as the typical imaging features on computed tomography and magnetic resonance (MR) imaging with MR cholangiography. Familiarity with various etiologies of sclerosing cholangitis and awareness of their typical clinical and imaging findings are essential for an accurate diagnosis and appropriate management.

Colangite aguda é uma infecção da árvore biliar que constitui uma emergência médica, sendo potencialmente fatal se não tratada imediatamente. Na maioria dos casos, ocorre devido à obstrução biliar por cálculos. Essa síndrome foi descrita pela primeira vez em 1877 por Charcot e o diagnóstico precoce pode oferecer melhor prognóstico.


Acute cholangitis is defined as infection of the bile ducts consisiting in a life-threatining emergency that should be treated immediately. It is mostly caused by common bile duct obstruction due to gallstones. This syndromme was first described by Charcot in 1877 and it must be promptly recognized in order to offer a better prognosis.

ABSTRACT

BACKGROUND/AIMS: We evaluated changes of causative pathogen in acute cholangitis and their antimicrobial susceptibility over six years and differences between community-acquired and hospital-acquired acute cholangitis at our institution. METHODS: Medical records of 1,596 patients with acute cholangitis and biliary drainage between August 2006 and August 2012 were reviewed retrospectively. Cases were divided according to time: period 1 (August 2006-December 2008, n=645, 40.4%), period 2 (January 2009-August 2012, n=951, 59.6%). Cases were divided according to community-acquired cholangitis (n=1,397, 87.5%) and hospital-acquired cholangitis (n=199, 12.5%). Causative pathogens and antimicrobial susceptibility were investigated in each group. RESULTS: Causative pathogen was isolated from bile culture in 1,520 out of 1,596 cases (95.2%). The three most frequently isolated Gram-negative bacteria were extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli (n=485, 30.4%), E. coli (n=237, 13.2%), and Citrobacter freundii (n=110, 6.9%). Between periods 1 and 2, prevalence of ESBL-producing E. coli and Klebsiella pneumoniae did not show significant change (36.7% vs. 32.1%, p=0.073; 6.6% vs. 6.2%, p=0.732). C. freundii showed a significant increase from period 1 to period 2 (1.7% vs. 13.2%, p=0.000). In both time periods, imipenem was the antimicrobial agent showing the highest rate of susceptibility (93.3% vs. 93.9%, p=0.783). Higher prevalence of ESBL-producing E. coli and C. freundii was observed in the hospital-acquired cholangitis group (52.1% vs. 31.2%, p=0.000; 15.9% vs. 7.3%, p=0.001). CONCLUSIONS: The most common causative pathogen of acute cholangitis was ESBL-producing E. coli. Prevalence of C. freundii increased over the time period. Imipenem should be reserved as an alternative for resistant pathogens.

A colangite aguda corresponde à infecção ascendente das vias biliares e geralmente está relacionada com as bactérias entéricas - gram-negativas, anaeróbios e enterococos. O fator patogênico mais importante é o obstrutivo, pois a presença de bactérias na bile não é suficiente para causar infecção das vias biliares se não estiver presente o componente obstrutivo. Dentre eles, a coledocolitíase é o principal elemento em cerca de 70% a 80% dos casos. Clinicamente, a tríade clássica - febre com calafrios, dor do hipocêndrio direito e icterícia - descrita por Charcot em 1877 manifesta-se em cerca de 70% a 80% dos pacientes. A ultrassonografia abdominal (e mais raramente a colangiopancreatografia por ressonância magnética) deve ser o método de escolha, por ser hábil em detectar dilatação da via biliar acima do local da obstrução e também por poder revelar sua causa. A terapêutica da colangite bacteriana aguda deve alicerçar-se na ressuscitação volêmica, antibioticoterapia e desobstrução da via biliar com consequente drenagem, sendo esta última a pedra angular da terapia emergencial da colangite aguda, que pode ser feita por via endoscópica (CPRE) ou transparietal (PTC).


Acute cholangitis corresponds to the ascending infection of the biliary tract and is usually associated with enteric bacteria - gram-negative, anaerobes and enterococci. The most important pathogenic factor is the obstruction because the presence of bacteria in bile is not sufficient to cause infection of the biliary tract in the absence of one obstructive component. Among these, choledocholithiasis is the main element in about 70% to 80% of cases. Clinically, the classic triad - fever with chills, pain in the right hypochondrium and jaundice - described by Charcot in 1877, manifests itself in about 70% to 80% of patients. The abdominal ultrasound (and, less frequently, magnetic resonance cholangiopancreatography) should be the method of choice because it is able to detect bile duct dilation above the site of the obstruction and also to reveal its cause. The treatment of acute bacterial cholangitis must be based on fluid resuscitation, antibiotics and clearing of the bile duct with subsequent drainage, the latter being the cornerstone of emergency therapy of acute cholangitis, which can be performed endoscopically (ERCP) or transparietal (PTC).

ABSTRACT

To study the diagnosis, therapy, precaution, and prognosis after surgical treatments of malignant changes that occurred in adult choledochal cysts. We analysed retrospectively the clinicopathologic data, the ways of operative treatment and the survival time of patients with malignancy in 74 cases of adult choledochal cysts in our hospital from 1986 to 2008. Among the 74 patients, 9 cases [3 males and 6 females] had carcinomas arising from choledochal cysts at the mean age of 51.6 +/- 16.4 years [range: 22 to 72 years] with the prevalence of 12.2% [9/74], which was significant difference comparing with no malignant change groups [P=0.0037]. The prevalence of malignancy for different groups were closely related to the age increased [r=0.363, p=0.011]: 3.4% for 16-30 group [1/29], 8.7% for 31-45 group [2/23], 15.4% for 46-60 group [2/13], and 44.4% for 61-75 group [4/9], respectively. Surgical treatments included cysts excision with Roux-en-Y hepaticojejunostomy in three patients, cysts excision with cholangiojejunostomy in two patients, partial cyst excision with left lobectomy and Roux-en-Y hepaticojejunostomy, pancreatoduodenectomy, chemotherapy with implantable drug delivery system via hepatic artery and portal vein, and choledochotomy with T-tube drainage and metastatic lymph node biopsy in one patient, respectively. The survival time was from three months to 66 months with the mean survival of 19.1 +/- 18.6 months. Choledochal cyst is a premalignant lesion and the incidence of malignancy increases remarkably with the increase in age. Patients require close monitoring so that recurrent carcinoma of the remnant bile duct can be identified as early as possible

ABSTRACT

We describe an unusual presentation of Clonorchis sinensis infection with obstructive jaundice due to duodenal papillitis which was relieved dramatically by endoscopic sphincterotomy. A 26-yr-old male presented with complaints of fatigue, weight loss and painless jaundice. The history was significant for frequent ingestion of raw freshwater fish. The patient underwent endoscopic retrograde cholangiopancreatography for evaluation of obstructive jaundice. The duodenal papilla was markedly edematous with a bulging configuration and hyperemic changes at the orifice. Cholangiography revealed mild bile duct dilatation and irregular wall changes with multiple indentations. However, there were no biliary stricture or stones noted as the cause of obstructive jaundice. We performed an endoscopic sphincterotomy for effective bile drainage through the duodenal papilla. After the sphincterotomy, the patient's jaundice was dramatically improved. Pathology of the duodenal papilla showed eosinophilic infiltration of the mucosa. Parasitic eggs, consistent with the diagnosis of C. sinensis, were found in the bile sample.

However, few studies have assessed its efficacy and safety in the very elderly. The purpose of this study was to evaluate the clinical outcomes of ERCP in the very elderly patients. METHODS: Eight hundreds two patients who underwent ERCP at Seoul National University Bundang hospital were enrolled retrospectively. They were divided into three groups according to their ages (non-elderly group, elderly group and very-elderly group; or =80, respectively). The indications and clinical outcomes including the complications of ERCP were compared among groups. RESULTS: The most common indication of ERCP was acute cholangitis in all the three groups. Periampullary diverticulum was more frequently observed in elderly and very-elderly patients than in younger patients. Mean duration of hospitalization was not different among three groups. ERCP success rate in all enrolled patients was approximately 90%, and there was no difference in terms of technical success rate between groups (p=0.1). However, the number of ERCP sessions was significantly higher in the very-elderly patients compared to in the non-elderly and elderly (1.38 vs. 1.13 and 1.18 respectively; p<0.001). There was no difference in mortality and complication rate between groups. CONCLUSIONS: ERCP can be performed safely in very-elderly patients. Therefore, only age should not be regarded as one of the major determining factors whether to perform ERCP.
